A new form needs completed each school year to remain in the program, but if you have already completed a form for the 2025-26 school year, you do not need to sign-up again. School Smiles accepts Medicaid, private insurance, and offers self-pay rates for those who may be uninsured.

Three Fremont gymnast, Natalei, Ruby and Jayde will be competing in the 2026 Indiana State Gymnastics meet this weekend in Indianapolis representing their club gymnastics team, Tri-State.
