What you MUST have completed before you can practice:

• IHSAA pre-participation physical (please make sure to complete all parts of all pages) – turned into the Athletic Office

• Your parent and yourself must read the information about concussions and about sudden cardiac arrest

• Acknowledgement parent/athlete signature form (concussions and sudden cardiac arrest) – signed by you and a parent and turned into the Athletic Office

• Acknowledgement parent/athlete signature form (athletic guidelines and expectations) – signed by you and a parent and turned into the Athletic Office
